Gynecomastia

Gynecomastia is a Greek word meaning woman like breasts. It is a common condition affecting 60-70 percent of males during their teenage years, mostly regressing spontaneously. In some individuals, gynecomastia can cause emotional discomfort and impair self-confidence. Some men may even avoid certain physical activities like gym, swimming, or going shirtless simply to hide their condition. The causes of common gynecomastia remain uncertain, although it has generally been attributed to an imbalance of sex hormones or the tissue responsiveness to them; a root cause is rarely determined for individual cases. Approximately 5-10% of cases of gynecomastia are due to drugs especially steroids, Aldactone (spironolactone), or some cardiac drugs like digoxin. Un-prescribed steroid intake by bodybuilders for rapid development of muscular mass is now a day commonly seen in this group of patients. Breast prominence can result from hypertrophy of breast tissue, chest adipose tissue (fat), and excessive skin, and is typically a combination. “There are many products (creams, gels, oils, slimming belts) available which claim to reduce the male breasts without surgery. These are just a marketing gimmick and a waste of money, health, and time. Be careful of such kinds of fake advertisements that claim to reshape your chest naturally.”

It is a day care surgery. The choices of Anesthesia include intravenous sedation and general anesthesia, Dr Goyal will recommend the best choice for you.
Modern gynecomastia surgery is a combination of excision of glandular tissue and lipo suction. Tumescent solution is infiltrated and liposuction of breast is performed. The cannula is moved back and forth in a controlled motion to loosen the excess fat, which is then removed from the body by vacuum suction.
Liposuction alone is not adequate in completely treating gynecomastia as the tough glands and fibrous tissue cannot be sucked out. After liposuction the remaining tissue is excised through a semicircular cut along the lower margin of areola which is the dark skin around the nipple.
After surgery, dressings or bandages will be applied to your incisions and you will be advised to wear an elastic bandage or support garment to minimize swelling and support your new chest contour as it heals. It should be worn 24 hours a day except during bath, for 2-3 months.

Your Recovery:

After the procedure, you will feel a little discomfort in raising your hands & feel mild stiffness and pain which will subside within 2-3 days. You will be given specific instruction to care the surgical site & medication to make you comfortable. All the normal office activities can be resumed after 2-3 days. On fourth day of surgery you can take shower and remove your dressing. Stitches are removed in 10 days. There is swelling, bruising and tenderness over the chest over the chest which goes down over next couple of weeks. You can do treadmill after 2 weeks while the heavy physical activities will be restricted for 4-6 weeks. A pressure garment is the most important thing after surgery, you will need to wear it immediately after the procedure for three months to minimize swelling & support your new chest contour.

Result

Final results are appreciable after 6-8 weeks with some more improvement over the next 6 months. Scars fade gradually over the next year. Results of gynecomastia Surgery are permanent, although subsequent obesity can create a gynecomastia-like effect.
